About the position The Remote Care Coordinator (LPN) position is a full-time role within the Chronic Care Management team, designed for licensed practical nurses who are looking to make a significant impact in patient care while enjoying the flexibility of remote work. This role involves providing support and education to patients enrolled in the Chronic Care Management program, monitoring their progress, and collaborating with physicians to ensure quality care. Responsibilities • Provide monthly calls and support to patients enrolled in the CCM program. , • Utilize nursing skills to educate patients and their families. , • Monitor patients' progress and work with physicians to ensure quality patient care.
