Hospital Liaison-Weekends-Remote Qualifications & Responsibilities... • Foster collaborative relationships with hospital discharge planners and medical professionals. • Conduct assessments and evaluations of potential residents in hospital settings. • Coordinate smooth transitions for patients from hospitals to our nursing home. • Educate healthcare providers and community partners about our facility's services and capabilities. • Act as a liaison between the nursing home and external healthcare entities. • Bachelor's degree in healthcare administration, business, or a related field (preferred). • 3-5 years Previous experience as an Admissions Director or Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital desired. • Strong communication and interpersonal skills. • Knowledge of healthcare regulations and standards.
