Quality Clinical Coordinator - Temporary (local remote - greater Philadelphia area) Tandigm Health, a transformational leader in population health management, is looking for a Quality Clinical Coordinator - Temporary in the greater Philadelphia area. We are looking for a Quality Clinical Coordinator (QCC) to join our Clinical team! You will be responsible for patient and practice outreach pertaining to medication adherence and Stars gaps. This includes coordinating and communicating with practice staff; researching patient demographic and clinical data, making outreach calls to patients, assisting with patient scheduling and other administrative tasks, and documenting outcomes. You are also responsible for other quality related functions that support the overall mission of the quality team by assisting with Stars/quality projects, working with practices as needed to support their quality work, and other administrative functions that are required for optimal team effectiveness. ESSENTIAL FUNCTIONS

  • Collaborates with the clinical quality team and practice staff to provide patient outreach and coordination with the highest level of professionalism and accountability.
  • Interacts with patients to encourage medication adherence, including exploring existing barriers, basic education, and facilitating necessary steps to positively influence medication adherence
  • Interacts with primary care practices and pharmacies to coordinate follow-up actions on behalf of patients.
  • At times, conduct patient outreach related to other preventive care, such as cancer screenings or diabetes care and coordinate patient screening or PCP follow up appointments as appropriate.
  • Documents patient interactions accurately and with great attention to detail, utilizing applicable platforms and Excel proficiently.
  • Communicates accurately, effectively, and in a timely manner with all stakeholders, including members of the Quality and the PHSO teams, Tandigm Medical Directors, PCPs and office staff, and other internal and external customers.
  • Develop familiarity with clinical and medical terminology pertinent to the patient population.
  • Helps to identify opportunities for collaboration with PCP, specialist, or clinical quality nurse for complex needs as well as ancillary community resources and services that can be leveraged in support of patients experiencing barriers to medication adherence and other Stars gaps.
  • Acts as a resource to others, as requested, resolving issues and responding to requests in a timely and efficient manner.
  • Provide education and support to PCP offices in developing effective and efficient processes for achieving 4+ Stars performance as appropriate.
  • Enter data into electronic medical records or quality platform as directed
  • Performs administrative functions to support the overall effectiveness of the quality team
  • Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
  • Performs additional duties, as assigned.

Required Qualifications: EDUCATION Minimum: High school diploma, G.E.D. or equivalent. Preferred: Associate’s or Bachelor’s Degree. EXPERIENCE Minimum: 3 years of professional experience in a physician practice; hospital; pharmacy or health plan. 3 years of professional experience in a customer-oriented setting. ESSENTIAL KNOWLEDGE, SKILLS, & ABILITIES:

  • Superior interpersonal skills; demonstrates the ability to engage and motivate patients, physicians and PCP office staff telephonically.
  • Organized, detailed, accurate.
  • Self-motivated and proactive; responds to requests in a timely and efficient manner.
  • Demonstrate success in project accountability and customer responsiveness.
  • Demonstrate sound decision-making skills and demonstrate the ability to work independently with supportive activities and interactions with both internal and external customers.
  • Customer-service oriented; responsive and respectful to internal and external customers.
  • Excellent communication skills; ability to represent the organization with skill and confidence.
  • Collaborative and resourceful; functions successfully in a team environment as well as independently.
  • Computer literate with proficiency in Microsoft Office suite of programs
  • Experience working in electronic medical records
  • Proficient with web-based search engines and software applications, office hardware (phones, fax machines, etc.)
  • Ability to type at least 45 wpm.
  • Knowledge and familiarity with medical and pharmacological terminology.
  • Knowledge and familiarity with Stars and HEDIS measures
  • Working knowledge of HMO/managed care/health plans
