What You’ll Do: ✔ Make an Impact Daily: 60+ outbound calls a day to engage and activate patients. ✔ Create Meaningful Connections: 19+ successful patient interactions per day. ✔ Coach & Motivate: Use motivational interviewing to drive positive health changes. ✔ Close Gaps, Change Outcomes: Help patients achieve SMART health goals and close HEDIS/care gaps. ✔ Identify & Provide Support: Assess Social Determinants of Health (SDOH) challenges and connect patients to community resources and tools that improve their well-being. ✔ Ensure High-Quality Care: Maintain a 90%+ QA score through effective communication. ✔ Celebrate Success: Capture and share real patient stories that inspire others. Who You Are: ✅ 3+ years of experience as a Medical Assistant (MA) ✅ Passionate about patient engagement & health coaching ✅ Strong in chronic disease management & care coordination ✅ Skilled at EHR/EMR navigation & documentation ✅ Empathetic, driven, and a problem-solver Why Join Us?
