Pharmacist- Pool

? The applicant would be expected to cover 1 Sunday shift (8 hours) per month. Other hours MAY be available. Essential Job Duties: ? Maintain thorough documentation of clinical activities which includes reviewing and verifying patient orders, conducting assessments and medication reconciliations, monitoring drug therapy outcomes, and providing consultations to healthcare professionals and patients regarding drug use and therapy adjustments. ? Supervise and participate in the preparation of sterile compounded medication, dispense medication and related supplies/equipment, and ensure compliance with federal, state, and local laws, as well as USP 797/800 standards for sterile compounding and hazardous drugs. ? Ability to interact and communicate effectively and positively with the pharmacy staff, all other Pentec departments, physicians and their office staff, clinics, and patients. The communication could include clinical functions, patient and operational meetings, patient scheduling and conference calls. ? Maintain clinical skills through continuous learning, assist in the clinical education of pharmacy students, participate in pharmacy quality commitment (PQC) activities, and support departmental operations including on-call rotations and obtaining necessary licensure. ? Other job duties and special projects as assigned. Ideal Candidate Will Have: ? B.S. or Pharm. D. from an accredited school of Pharmacy. Licensure in good standing to practice pharmacy in the state in which the pharmacy is physically located. ? Experience in clinical management of home infusion therapy patients. Experience in sterile compounding including knowledge of USP 797/800 guidelines. ? Excellent communication and interpersonal skills, problem solving abilities, ability to prioritize and handle multiple tasks concurrently, ability to understand and maintain principles of confidentiality and HIPAA.
